Programs / Center of Excellence in Primary Health Care
The US National Academy of Medicine (1) and World Bank (2) reports conclude that team-based, community-oriented Primary Health Care (PHC) that includes disease prevention, health education, and care of NCDs will improve health outcomes. This strategy is essential to achieve universal access to health services.
These expert groups support conducting demonstration projects to show the clinical and financial value of this approach. In line with these recommendations and the National priorities LeBoHA established a Center of Excellence in Team-Based, Community-Oriented PHC to care for patients at Motebang hospital, in the Leribe district.
This COE is designed to implement a population health approach to health education, preventive services and wellness; provide continuity of care for non-communicable diseases; and collect data to demonstrate the clinical and financial value.
Patients who are identified by COE clinicians or via referral from Motebang OPD or inpatient wards who have advanced and/or complex chronic medical condition(s). Specific clinical criteria (e.g. blood pressure and glycemic values) have been delineated.
Patients who have complex medical needs similar to those in Category 1 but who are not enrolled in COE and instead benefit from telehealth consultation between their health center or OPD provider and a COE Family Medicine or Pediatrics specialist.
